The formula you need is D=m/v, where D is the density, m the mass and v the volume.

1ml = 1cm3

so 12ml = 12cm3

D (in this case) = 4.05/12 = 0.3375 g/cm3

Density is mass per unit volume, so mathematically, it's D=M/V. If your mass is 4.5 grams and your volume is 5 ml, then density is 4.5/5=0.9 g/ml.
