This form is used in hospitals. It means that if a patient stops breathing, nurses and doctors will not attempt to revive the patient. This form is usually used when a patient is extremely ill and attempting to resuscitate them would only prolong their suffering. The form is signed by the patient, or someone with power of attorney on behalf of the patient if they are too ill to sign it themselves.
