A sphere of radius 4000 miles has a circumference of 25132 miles. The distance between any two points on the surface of that sphere is simply 25132 times the angle between the two points divided by the angle of the full circle (360 degrees). If the two cities were ten degrees apart, the distance would be 698 miles. It does not matter if the two cities are on the same meridian or not - angular separation is the only thing that matters, except that the calculation of the angle in three-space is more complex than that in two-space.
