The signal for offside in soccer is when the lineman raises his flag above his head and stops running in line with the last defender. When the referee has realized the linesman has raised his flag the linesman points the flag out in front of him.

It depends on the level of the game. Youth games have 2 to 4 officials working those games. They try to schedule 3 or 4. High school: Freshamn have 3 officials working those game. (Referee, Umpire and Head Linesman) J.V. has 4 officials working those games. (Referee, Umpire, Line judge and Head Linesman) Varsity has 5 officials working those games. (Referee, Umpire, Line Judge, Head Linesman and Back Judge) College and NFL games have 7 officials working those games (same as High school varsity, but also with Field Judge and Side Judge)
The referee that stands in the middle of the field is the referee who is the final authority for that game. All decisions made by this referee are final, regardless of what is done or said by any other player, coach, spectator, or assistant referee. Generally, this position is referred to as the "center referee," or some other similar nomenclature. I have heard "head referee," "center official," "middle referee," or "the dude in the center of the field."
