This means switches and other devices with operable controls go 48 inches AFF (above finished floor), measured to the switch handle (which means to the center of the box). This is the maximum height permitted by ADA. Receptacles don't seem to have a real standard, I have seen from 12 to 18 inches where I work. If there are existing recepts in the building, we match those. At my house, for instance, they are set 12" to the top of the box.
