Usually an insurer will apply a provision called LEAT--Least Expensive Alternative Treatment--to the payment of a composite filling that is placed on a molar or other tooth where the coverage is for amalgam fillings. This means that they will pay as if the composite filling was an amalgam one. Here is an example. Say an amalgam two surface filling on a molar costs $150 but a two surface composite filling costs $220.
