Draw 6 congruent rectangles. 4 of them should be in a straight line. Label them 1 to 4 from left to right. Draw the other two congruent rectangles facing the same direction on either side of face 2, labeled 5 and 6. Cut out the figure along the outside edges. Fold face 1 up so that it is perpendicular to face 2. Fold faces 5 and 6 up so that they are parallel to each other, and perpendicular to faces 1 and 2. Fold face 3 up so that it is parallel to face 1, and fold face 4 over so that it becomes the top of the prism. Tape along the edges between: 1 and 5, 1 and 6, 3 and 5, 3 and 6, 4 and 1, 4 and 5, 4 and 6.
