The meaning of the phrase "not to scale" is to let you know that the drawing, map, etc. does not represent the real world lengths or measures.

In other words, if you were to look at a map with 3 towns on it, you couldn't use the visual distance to determine which one was closer. Similarly, the photograph of a product (like a plastic bead) which is on the box is not representing the true size of the object.
