The second hand goes around once.
A normal clock has twelve hours and it goes through two cycles of that period every day. There are occasions when you need to know whether it the time referred to is in the fist cycle (midnight to noon = am) or the second (noon to midnight = pm).

It's the opposite direction of the normal rotation of the hands of a clock. Looking at a clock face, the hands travel over the numbers 1,2,3,4 etc - counter-clockwise they's move in the reverse direction - 4,3,2,1 etc.
