Though some people may believe that Johannesburg, South Africa may be the most violent city in the world, it's not.
The most violent city in the world is actually the capital of Venezuela, Caracas; which is also the country's largest city. The city of 3.2 million has a homicide rate of 130 for every 100,000 residents per year on average.
To put that in better context the city of New York has a population of 8,274,527 people. If New York had the same homicide rate as Caracas then about 10, 757 people would be murdered every year.
UPDATE: The most violent city in the world is now Ciudad Juárez, Mexico, with 191 homicides per 100,000 residents in 2009.
