If you are tring to get "x" alone, you would do this:

y=x+4

y-4=x

If you are trying to get "y" alone, then y=x+4 is your answer.

Y and X represent the 'Unknown Number'. For example, Y might mean the number 6 and X means the number 2, so then Y equals the same amount as X + 4 does which, in my example, is 6 but the numbers value to the letters (what the letter replacing the number is) changes from equasion to equasion. Generaly, somewhere written on the sheet it may say Y = 6 so then you would have to find out what X means, which in my example, is 2.
