A single "form-C contact" means a set of three terminals, where the middle one connects to

one of the ends when the relay is energized, and connects to the other one when the relay

is de-energized.

"3 form-C" means there are three separate individual sets of form-C contacts on the same

relay. There's no connection between the sets, but when the relay is switched on or off,

they all flip from one side to the other at the same time.
