Nowadays, middle school usually contains the grades of 6th-8th. There are some middle schools who still follow the olden day rules of containing grades 7th-8th, or even 7th-9th.

There is not one set "gifted grade". If a student is performing well above what is required for their certain grade, then they can be moved up to gifted grades, including 5th grade.
What grades are in middle school depends upon the school district. It is the 2 or 3 grades between elementary school and high school. It includes 7th & 8th grades, but might also have either the 6th or 9th grades. The school district where I grew up had junior high schools with 7th, 8th and 9th grades. They still do, but have changed to calling them "middle school". Where my children grew up, middle school was, and still is, 7th and 8th grades.
The highest grade is 8, but then you can do Diplomas. The Diplomas include AMusA, LMusA and FMusA. Most people who are serious about learning piano complete up to grade 8 if they play piano for their own enjoyment. Many people learn up to AMusA because that is the grade you have to pass to be able to teach piano grades.
